Have you lost or misplaced your CERTIFICATE OF COMPLETION? If so, you can REQUEST an additional copy for a $10 fee. 

1. A SUBMISSION FORM is REQUIRED to process your CERTIFICATE OF COMPLETION.

2. Confirm that all entered information is correct before submitting your SUBMISSION FORM. 

3. Once we receive your payment confirmation, your CERTIFICATE OF COMPLETION will be emailed to the provided email address within 24 hours.

4. You will receive a complimentary TEXT MESSSAGE to the provided contact number to inform you to check your email inbox once your CERTIFICATE OF COMPLETION has been emailed.
