IMPORTANT INSTRUCTIONS:

This form is for licensees who have purchased a CE course only.
If you purchased multiple CE courses, please submit this form once for each course using the same order number.
Each course requires a separate Submission Form.
Once you complete and submit your Submission form, your Certificate of Completion will be emailed to the provided email address from your submission form.
When you receive your Certificate of Completion, SAVE IT because licensees are now responsible for uploading your Certificate of Completion to CE Broker and self-report your CE hours to complete the license renewal and/or reinstatement process.
The CEU Academy does not submit CE hours to CE Broker on behalf of licensees.
Who is CE Broker? CE Broker is the CE Hour Tracking System used by the Georgia Secretary of State for license renewals and/or reinstatement.
